Clerk of the Year Nomination Form (This form is used to nominate a "Clerk of the Year" . Complete and submit to the Chairman of the Clerk of the Year Committee at the appropriate time as indicated on the form.) Committee Interest Form (This form is used to become appointed to a Lone Star Chapter Committee. Complete and forward to President Kathy Wingo) Member Of the Year Nomination Form (This form is to be completed to nominate a "Member of the Year". Complete and submit to the Chairman of the Clerk of the Year Committee at the appropriate time as indicated on the form. ) Chapter Membership Application (This form is to be completed to become a member of the Lone Star Chapter. Please complete and return to Treasurer Diane Zucco) Nomination of Officer Form (This form is to be used to nominate a member as an officer of the Lone Star Chapter. Complete and return to the Chairman of the Nominating Committee at the appropriate time as indicated on the form.) Scholarship Application (This form is to be completed and submitted to the Chairman of the Scholarship Committee. This form must be completed and submitted no later than March 1 of each year.) |
